1. Immunization Requirements - “California law requires that each child entering school for the first time present evidence (unless exempt for personal or medical reasons) that he or she has been protected against polio, diphtheria, pertussis (whooping cough), tetanus (lockjaw), measles, rubella (German measles), mumps, hepatitis B, and varicella (chicken pox).”* Proof of freedom from active tuberculosis (skin test or x-ray) is also required in Orange County.

2. Physical Examination for Entry - All entering first grade students must have a physical examination and will be certified (or waived) for school entry in writing.** (Your family physician has the form or you may obtain one fro the school office.)
